Please explain the basis on which you are entitled to work in the UK, i.e. UK citizen, EU citizen. Indefinite leave to remain, please put all expiry dates for right to work if applicable.
Please also note that regular drug and alcohol tests are performed throughout the period of the assignment. A test will also be performed by hiring companies once the 12 week period is completed and contract offered. Any failure of these tests WILL result in the instant dismissal on grounds of health and safety.
